Job summary

Gentiva is seeking an Afterhours RN Care Manager to provide high-quality hospice care during nights, weekends, and on-call hours. This role ensures seamless coverage, maintains clinical oversight, and collaborates with physicians and the IDG for coordinated patient care.

The ideal candidate will hold an active RN license, have at least one year of relevant RN experience, and be proficient with EMR systems and patient education.

Qualifications

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum one year RN experience in acute care, oncology, long-term care, hospice, or home health.

Responsibilities

• Provide direct in-person patient care during on-call, weekend, and after-hours shifts.
• Assess physical, emotional, psychosocial, and spiritual needs.
• Support the IDG process with chart reviews and plan of care updates.
• Educate patients and families on disease progression and symptom management.
• Maintain timely clinical documentation and care plans.
