Job summary

Gentiva Health Services, Inc. seeks an Afterhours RN Care Manager to provide high-quality hospice care outside standard hours. You will assess, coordinate, and oversee care for patients and families during nights, weekends, and on-call periods, acting as a key link in the IDG to ensure seamless service delivery.

The role requires an active RN license and at least one year of relevant RN experience. A $7,500 sign-on bonus is offered along with comprehensive benefits and professional development

Qualifications

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum one year of RN experience in acute care, oncology, long-term care, hospice, or home health.
• Must meet any state-specific requirements.

Responsibilities

• Provide direct in-person care during on-call, weekend, and after-hours shifts.
• Assess physical, emotional, psychosocial, and spiritual needs.
• Support the IDG process through chart reviews, plan of care updates, and interdisciplinary communication.
• Develop and revise the Hospice Plan of Care in collaboration with physicians and IDG members.
• Educate patients and families on disease progression, symptom management, medications, and hospice philosophy.
