Job summary

Gentiva Hospice seeks an Afterhours RN Care Manager to provide high-quality hospice care outside standard hours. You will assess and coordinate patient and family needs during nights and weekends, maintaining clinical oversight for assigned patients and serving on the IDG for cross-functional coordination.

Responsibilities include on-call visits, plan of care updates, education for families, and strict documentation.

Qualifications

• Strong knowledge of hospice principles and regulations.
• Ability to conduct thorough patient assessments and care planning.
• Effective communication with patients, families, physicians, and IDG members.

Responsibilities

• Provide direct in-person care during on-call, weekend, and after-hours shifts.
• Assess physical, emotional, psychosocial, and spiritual needs.
• Educate patients and families on disease progression and hospice philosophy.
• Maintain accurate clinical documentation and plans of care.
• Coordinate with IDG and physicians on care updates.
