Gentiva is seeking an Afterhours RN Care Manager to provide high-quality hospice care outside standard hours in Washington Court House, OH. The role includes on-call nights, weekends, and coordination with the IDG to ensure seamless care.

You will assess patient needs, manage plans of care, educate families, and maintain timely documentation while upholding hospice principles and compliance standards.

Qualifications

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum one year of RN experience in acute care, oncology, long-term care, hospice, or home health.
• Must meet any state-specific requirements.

Responsabilités

• Provide direct in-person patient care during on-call, weekend, and after-hours shifts, including assessments, admissions, routine visits, death visits, and live discharges.
