Gentiva Hospice is seeking an Afterhours RN Care Manager to provide high-quality hospice care outside standard business hours. The role ensures seamless coverage, continuity of care, and strong clinical management for patients and families during nights, weekends, and on-call hours.

The RN will assess needs, coordinate care, participate in IDG, and maintain primary clinical oversight while adhering to hospice standards and regulatory requirements.

Qualifications

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum one year of RN experience in acute care, oncology, long-term care, hospice, or home health.
• Must meet any state-specific requirements.

Responsibilities

• Assess and coordinate patient and family needs after-hours.
• Provide direct in-person care during after-hours and weekends.
• Maintain primary clinical oversight for assigned patients.
• Educate patients and families on disease progression and hospice care.
• Document assessments and care plans in the EMR.
• Collaborate with the IDG and physicians on care plans.
