Gentiva Hospice is seeking an After Hours RN to respond to patient needs during evenings, nights, weekends, and holidays, delivering compassionate care when it is most needed.

You will assess patients, coordinate with the hospice team, educate families on care plans, and maintain accurate records while ensuring continuity of care. This role emphasizes independent work and strong clinical judgment in a hospice setting.

Qualifications

• Current RN license in the state of employment or eligibility to obtain.
• Minimum 1 year of nursing experience.
• Compassionate, calm demeanor in urgent care situations.
• Excellent clinical judgment for handling emergencies with patients.
• Comfort delivering care in patients' homes, facilities, or other settings.
• Valid driver''s license, automobile insurance, and reliable transportation.
• Current CPR certification.

Responsabilités

• Provide direct patient care and support after normal business hours.
• Serve as the primary liaison between patients, families, physicians, and the hospice team during your shift.
• Evaluate patient and family needs and implement the plan of care.
• Document clinical visits and communications in the EMR.
• Collaborate with the interdisciplinary group including LPNs/LVNs, CNAs, and other staff.
• Maintain accurate and updated medical records and plans of care.
• Respond to urgent calls, patient visits, and symptom management needs.
• Participate in quality assessment and performance improvement programs.
• Work independently while coordinating with the daytime care team.
