Job summary

Gentiva seeks an experienced RN Care Manager to provide after-hours on-call hospice care and clinical oversight for assigned patients. You will conduct comprehensive assessments, coordinate with physicians and the interdisciplinary group, and ensure timely documentation and plan-of-care updates.

The role requires strong hospice knowledge, leadership in field staff, and adherence to infection control and regulatory standards. Location is Mineral Wells, TX, with full-time hours.

Qualifications

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum one year of RN experience in acute care, oncology, long-term care, hospice, or home health.

Responsibilities

• Provide direct in-person patient care during on-call, weekend, and after-hours shifts.
• Assess physical, emotional, psychosocial, and spiritual needs.
• Educate patients and families on disease progression and hospice care.
• Maintain accurate clinical documentation and care plans.
• Coordinate with IDG and physicians; update plans of care.
• Ensure infection control and safety standards.
