Job summary

Banyan Treatment Centers is seeking an Aftercare Coordinator in Lake Worth, FL to manage discharge planning, continuity of care, and collaboration with clinical staff, families, and external partners.

This role ensures patients leave treatment with a clear aftercare plan, with a focus on long‑term recovery and preventing gaps in care. The position emphasizes documentation, team coordination, and patient safety, within a mission‑driven organization.

Qualifications

• Experience in discharge planning and care coordination.
• Strong written and verbal communication.
• Familiar with behavioral health services.
• Knowledge of EMR systems (Salesforce or Kipu preferred).

Responsibilities

• Coordinate discharge plans and next level of care.
• Obtain consents, IDs, and financial documents.
• Refer discharged patients to appropriate providers.
• Serve as liaison with VA, probation officers, attorneys, HR, and EAPs.
• Maintain EMR documentation and coordinate using Teams and Salesforce.
• Participate in weekly case consultations.
