Job summary

Banyan Treatment Centers is seeking an Aftercare Coordinator to support patients transitioning through treatment, focusing on discharge planning and continuity of care. This role collaborates with clinical teams, families, and external partners to ensure a clear, well-coordinated aftercare plan.

The position emphasizes preventing gaps in care and addressing post-treatment needs with compassion, reporting to the Executive Director, and operating on-site in Laurel Run, PA, with a full-time

Qualifications

• High school diploma or GED required; Associate’s degree preferred.
• Experience in behavioral health, customer service, or familiarity with 12-step or recovery-based programs preferred.
• Strong written and verbal communication skills with attention to detail.
• Familiarity with Salesforce and/or Kipu EMR preferred.
• Knowledge of local community resources and aftercare support services.

Responsibilities

• Coordinate discharge plans and next level of care with clinical staff to ensure seamless transitions.
• Obtain consents, IDs, and financial documents to support continuity of care.
• Refer discharged patients to appropriate providers when necessary.
• Serve as liaison with external agencies and partners (e.g., VA, probation officers, HR, EAPs).
• Communicate with referral sources about patient arrivals, AMA risks, and hospitalizations.
