Banyan Treatment Centers is seeking a dedicated Aftercare Coordinator to support patients as they transition through every stage of their treatment journey. The role focuses on discharge planning, continuity of care, and collaboration with internal teams, families, and external partners to ensure each patient leaves treatment with a clear, well-coordinated aftercare plan.

Qualifications

• Requires high school diploma or GED; an associate’s degree is preferred.
• Experience in behavioral health or recovery-based programs is preferred.
• Strong written and verbal communication with attention to detail.
• Familiarity with Salesforce and Kipu EMR is preferred.

Responsabilités

• Coordinate discharge plans and next level of care with clinical staff to ensure seamless transitions.
• Obtain consents, IDs, and financial documents to support continuity of care.
• Refer discharged patients to appropriate providers when needed.
• Serve as liaison with external agencies and partners (e.g., VA, probation, attorneys, HR, EAPs).
• Communicate with referral sources about patient arrivals, AMA risks, and hospitalizations.
