Job summary

Banyan Treatment Centers is seeking a dedicated and organized Aftercare Coordinator to support patients as they transition through every stage of their treatment journey. This part-time role focuses on discharge planning, continuity of care, and collaboration with internal teams, families, and external partners to ensure each patient leaves treatment with a clear aftercare plan.

Qualifications

• Must have a high school diploma or GED; associate degree preferred.
• Experience in behavioral health or customer service preferred.
• Strong written and verbal communication skills.
• Familiarity with Salesforce and/or Kipu EMR preferred.
• Knowledge of local community resources.

Responsibilities

• Coordinate discharge planning and continuity of care with clinical staff and outbound team to ensure seamless transitions.
• Assist with admissions-related needs by obtaining consents, IDs, and financial documentation.
• Refer administratively discharged patients to appropriate providers when necessary.
• Serve as liaison with external agencies and partners (e.g., VA, probation officers, attorneys, HR, EAPs).
• Communicate with referral sources about patient arrivals, discharge planning, AMA risks, and hospitalizations.
• Engage families throughout the discharge process with timely updates and alignment with aftercare planning.
• Maintain accurate documentation in EMR (Kipu) and coordinate using Teams and Salesforce.
• Support retention and safety initiatives, including AMA blocking efforts.
• Participate in weekly case consultations with clinical, operational, and business development teams.
