Job summary

Banyan Treatment Centers in Castle Rock, CO invites an Aftercare Coordinator to manage discharge planning and continuity of care. You will collaborate with clinical staff, families, and external partners to ensure a clear, well-coordinated aftercare plan for each patient.

This full-time role focuses on coordination across teams, documentation in Kipu EMR, and effective liaison with external agencies to support long-term recovery outcomes.

Qualifications

• High school diploma or GED required; an associate degree is preferred.
• Experience in behavioral health or customer service is preferred.
• Strong written and verbal communication skills with attention to detail.
• Familiarity with Salesforce and/or Kipu EMR is preferred.
• Knowledge of local community resources and aftercare support services.

Responsibilities

• Coordinate discharge plans and next levels of care with clinical staff to ensure seamless transitions.
• Obtain consents, IDs, and financial documents to support continuity of care.
• Refer administratively discharged patients to appropriate providers when needed.
• Serve as a liaison with external agencies and partners (VA, probation, attorneys, EAPs).
• Communicate with referral sources about arrivals, discharge planning, AMA risks, and hospitalizations.
• Engage families during discharge and align with aftercare planning.
• Maintain accurate documentation in the EMR (Kipu) and share updates using Teams/Salesforce.
• Support patient retention and safety initiatives, including AMA blocking efforts.
• Participate in weekly case consultations with clinical, operational, and business teams.
• Uphold confidentiality and regulatory requirements with a patient-first approach.
