Banyan Treatment Center in Castle Rock, CO seeks an Aftercare Coordinator to guide patients through discharge planning and the next level of care. You will coordinate with clinical staff, families, and external partners to ensure a clear, well-coordinated aftercare plan that supports long-term recovery.

Qualifications

• High school diploma or GED required; associate degree preferred.
• Experience in behavioral health or recovery-focused programs is a plus.
• Strong written and verbal communication skills with attention to detail.

Responsabilités

• Coordinate discharge plans and next level of care with clinical staff and Banyan’s Outbound Team.
• Obtain consents, IDs, and financial documentation to support continuity of care.
• Refer discharged patients to appropriate providers when needed.
• Act as liaison with external agencies and partners (VA, probation, attorneys, HR, EAPs).
• Communicate with referral sources about patient arrivals, AMA risks, hospitalizations, and continuity of care.
• Engage families during discharge, providing timely updates and alignment with aftercare planning.
• Maintain accurate EMR documentation in Kipu; share updates via Teams and Salesforce.
• Support patient retention and safety initiatives, including AMA blocking efforts.
• Participate in weekly case consultations and collaborate with clinical, operational, and business teams.
• Uphold confidentiality and patient-first approach in all communications.
