Gentiva is seeking an After Hours Hospice RN to respond to patient needs during evenings, nights, weekends, and holidays, delivering compassionate care where it is needed most. You will work directly with patients and families, coordinating with physicians and the hospice team to manage symptoms and plan care.

Requirements include an active SC RN license (or eligibility), 1+ year nursing experience (hospice/home health preferred), CPR, and reliable transportation.

Qualifikationen

• Current RN license in SC or eligibility to obtain.
• 1+ years of nursing, hospice or home health preferred.
• Excellent clinical judgment and compassionate communication.
• Valid driver’s license and reliable transportation.
• CPR certification current.

Aufgaben

• Provide direct patient care after normal hours.
• Serve as liaison among patients, families, physicians, and hospice team.
• Document visits and communications accurately in EMR.
• Respond to urgent calls and manage symptoms.
• Collaborate with LPNs/LVNs, CNAs, and support staff.
