Job summary

Gentiva Hospice is seeking an After Hours / On-Call Registered Nurse to respond to patient needs during evenings, nights, weekends, and holidays. The role requires 1+ year of hospice experience and an active Texas RN license.

You will work seven days on, seven days off, delivering compassionate, home-based care and coordinating with the daytime team. Responsibilities include direct patient care after hours, urgent symptom management, and documentation in the EMR.

Qualifications

• 1+ year of hospice experience
• Current RN license in the state of employment
• CPR certification
• Valid driver’s license and reliable transportation
• Excellent clinical judgment and ability to work in emotionally intense settings

Responsibilities

• Respond to patient needs after normal business hours
• Provide direct patient care and support during evenings, nights, weekends, and holidays
• Serve as liaison between patients, families, physicians, and hospice team
• Document visits and communications accurately
• Coordinate with daytime care team to ensure continuity of care
• Participate in quality assessment and performance improvement programs
