Job summary

Gentiva Hospice is seeking an After Hours RN to deliver compassionate care in patients' homes during evenings, nights, weekends, and holidays. You will respond to urgent needs, collaborate with the hospice team, and ensure the plan of care is followed with strong documentation and coordination.

The role requires an active RN license in Illinois, CPR certification, and 1+ year of nursing experience. This is a safety-sensitive, full-time position with comprehensive benefits.

Qualifications

• Current RN license in Illinois as required for employment.
• Minimum 1+ year of nursing experience.
• CPR certification required; strong clinical judgment.

Responsibilities

• Provide direct patient care and support after normal business hours.
• Serve as primary liaison between patients, families, physicians and the hospice team.
• Respond promptly to urgent calls, patient visits and symptom management needs.
• Evaluate patient and family needs to guide clinical decisions and plan of care.
• Document clinical visits and communications accurately in EMR.
• Collaborate with interdisciplinary group to deliver holistic care.
