Job summary

Gentiva Hospice is seeking an After Hours/On-Call Hospice RN to respond to patient needs during evenings, nights, weekends, and holidays, delivering compassionate care when it’s needed most.

You’ll report to the Administrator or Patient Care Manager Senior, provide direct care after hours, coordinate with the interdisciplinary team, document visits, and participate in quality improvement.

Qualifications

• Current RN license in the state of employment (or eligibility to obtain).
• At least 1 year of nursing experience.
• Hospice heart: compassionate, calm, and dependable in urgent care situations.
• Valid driver’s license and reliable transportation.
• Current CPR certification.

Responsibilities

• Provide direct patient care after hours and serve as liaison among patient, family, physicians, and hospice team.
• Document visits and communications accurately and participate in QAPI program.
• Assess patient and family needs to guide the plan of care.
• Coordinate with interdisciplinary team (LPNs/LVNs, CNAs) to ensure continuity of care.
