Gentiva is seeking an After Hours Hospice RN to respond to patient needs during evenings, nights, weekends, and holidays, delivering compassionate care when it’s needed most. Schedule includes Saturday and Sunday 8am-8pm, plus one weekday.

You will report to the Administrator or Patient Care Manager Senior and serve as liaison among patients, families, physicians, and the hospice team, ensuring timely visits, symptom management, and care coordination.

Formación

• Current RN license in the requested state or eligibility to obtain.
• 1+ years of nursing experience.
• Compassionate, calm, and dependable in urgent care situations.
• Excellent clinical judgment and EMR documentation skills.
• Comfortable providing care in patients' homes or facilities.

Responsabilidades

• Respond to patient needs during evenings, nights, weekends, and holidays.
• Serve as primary liaison between patients, families, physicians and hospice team.
• Assess and manage symptoms, document visits and communications.
• Coordinate with interdisciplinary team to ensure continuity of care.
• Participate in quality assessment and performance improvement programs.
• Maintain infection control and safety standards.
