Job description

Job Title: Physical Therapist (PT) - Home Health

BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for a full-time opportunity providing home health visits at our senior living office in Gaithersburg, MD. This office serves adult and geriatric clients across Montgomery County.

Requirements:

1. One year of clinical experience as a licensed Physical Therapist.
2. Prior home health experience is a plus.

Role Overview: As a home care Physical Therapist, you will be an integral part of a multidisciplinary team providing rehabilitative care and skilled nursing to help clients remain at home.

Responsibilities:

1. Make home visits to clients in designated areas, focusing on Start of Care visits.
2. Perform diagnostic tests such as mobility, joint range of motion, stability, ambulation patterns, strength, endurance, and safety assessments.
3. Implement wound prevention and repair interventions using evidence-based dressings and topical agents.
4. Develop and revise individualized care plans including therapeutic exercises, gait training, and balance interventions.
5. Assess and update care plans, collaborating with nursing staff as needed.
6. Educate clients and their families on rehabilitative activities to promote health and independence.
7. Document all observations, interventions, and evaluations accurately using electronic medical records on a tablet.

Qualifications:

1. Current Maryland Physical Therapist license.
2. Minimum of one year recent experience as a PT.
3. Graduation from an APTA-approved program or equivalent.
4. Ability to work independently and manage time effectively.
5. Strong interpersonal, organizational, and problem-solving skills.
6. Proficiency with computers; experience with EMR preferred.
