JOB DESCRIPTION
CH Home North Metro B2 - Home Care PT
Traveler can expect to be onsite for orientation for approx. 2 weeks with the first day in Oak Brook.
Then the position is 100% remote within the designated coverage area covering approx. 170 patient census. Patients are adult/geriatric general med/surg type patients. Expectation is for 5-6 patient visits per day within coverage area.
There is not a physical office they will report to, all meetings are held virtually. Supplies and equipment will be delivered to traveler’s home.
Candidate MUST provide their own transportation to/from all patient visits.
Any solid color scrubs acceptable.
Coverage area: Lake Forest, Highland Park, Lincolnshire, Libertyville, Mundelein, Vernon Hills, Grayslake.
Candidate MUST HAVE:
- 2 years Home Health PT experience
- Master's Degree in PT
- active PT license (IL) and BLS (AHA)
- experience with adult and geriatric patients
- able to assist with ADL's and adaptive equipment
- valid Driver's License, Auto Insurance, and reliable transportation
- able to effectively collaborate with other specialties (RN, OT, Speech, etc.).
