Job Opportunity: Physical Therapist (PT) at BAYADA Home Health Care

BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for part-time or per diem home health visits serving adult and geriatric clients in Frederick County.

As a home care Physical Therapist, you will be a vital part of a multidisciplinary healthcare team providing rehabilitative care and skilled nursing, enabling clients to remain at home.

Responsibilities:

1. Make home visits to clients within designated territories.
2. Perform diagnostic tests and assessments related to mobility, strength, balance, and safety.
3. Develop and implement individualized care plans with therapeutic exercises and interventions.
4. Assess and update care plans, collaborating with nursing staff as needed.
5. Educate clients and families on rehabilitative activities to promote health and independence.
6. Document all observations and interventions accurately using electronic medical records.

Qualifications:

1. Current Maryland Physical Therapist (PT) license.
2. Graduation from an accredited physical therapy program.
3. Ability to work independently and manage time effectively.
4. Strong interpersonal and organizational skills.
5. Proficiency with computers; prior EMR experience preferred.
