BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for a full-time outpatient home health position. The role involves home visits for senior living clients in Montgomery County, primarily in independent and assisted living communities.

Role Overview

As a home care Physical Therapist, you will be part of a multidisciplinary team providing rehabilitative and skilled nursing care, enabling clients to remain safely at home.

Responsibilities

• Conduct outpatient visits within designated areas.
• Perform diagnostic assessments including mobility, strength, balance, and safety evaluations.
• Create and update personalized care plans with interventions such as manual therapy, gait training, and balance exercises.
• Assess and modify care plans as needed, collaborating with nursing staff.
• Educate clients and families on rehabilitative activities to promote health and independence.
• Document all observations and interventions accurately using electronic tablets.

Qualifications

• Current Maryland Physical Therapist (PT) license.
• At least one year of recent PT experience.
• Graduation from an American Physical Therapy Association-approved program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficiency with computers; EMR experience preferred.
