Job description

BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for part-time or per diem home health visits for adult and geriatric clients in Washington County.

As a home care Physical Therapist, you will be a vital part of a multidisciplinary team providing rehabilitative and skilled nursing care, enabling clients to remain at home.

Responsibilities:

1. Make home visits to clients in designated areas.
2. Perform diagnostic tests, including mobility, strength, balance, and safety assessments.
3. Develop and implement individualized care plans with manual therapy, gait training, and other interventions.
4. Assess and update care plans regularly, collaborating with nursing staff.
5. Educate clients and families on rehabilitative activities to promote health and safety.
6. Document observations and interventions accurately using electronic tablets.

Qualifications:

• Current Maryland Physical Therapist (PT) license.
• Graduation from an APTA-approved program or equivalent.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficiency with computers; experience with EMR preferred.
