Job description

BAYADA Home Health Care is currently seeking an experienced Physical Therapist (PT) for a full-time opportunity performing home health visits for our senior living office. This office services adult and geriatric clients on a per-visit basis in territories throughout the Arlington/Alexandria, VA area.

As a home care Physical Therapist, you will be an integral member of a multidisciplinary healthcare team that provides rehabilitative care and skilled nursing to clients, enabling them to receive necessary medical care to remain at home.

Responsibilities include:

1. Make home visits to clients in designated territories.
2. Perform diagnostic tests and measurements, including mobility, range of joints, transfer status, stability, ambulation patterns, muscle strength and endurance, balance testing, and safety assessments.
3. Implement integumentary protective interventions to prevent wound formation and promote wound healing using evidence-based dressings and topical agents.
4. Develop and revise individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
5. Assess and update the care plan regularly, collaborating with nursing staff as part of the multidisciplinary team.
6. Educate clients, family members, or representatives on rehabilitative care and activities to promote health, safety, and independence.
7. Accurately document observations, interventions, and evaluations using a state-of-the-art touchpad tablet.

Qualifications:

1. Current VA Physical Therapist (PT) license.
2. Graduation from an American Physical Therapy Association-approved program or equivalent, evidenced by transcript or diploma.
3. Ability to work independently and manage time effectively.
4. Strong interpersonal, organizational, and problem-solving skills.
5. Proficiency with computers; experience with electronic medical records (EMR) preferred.
