Job Opportunity: Outpatient Physical Therapist (PT) at BAYADA Home Health Care

BAYADA Home Health Care is seeking an experienced Outpatient Physical Therapist (PT) for full-time outpatient visits in assisted living communities through our Senior Living office located in Falls Church, VA. This role involves servicing adult and geriatric clients across territories in Alexandria/Arlington, VA.

As a Physical Therapist, you will be a vital part of a multidisciplinary healthcare team, providing outpatient services that enable clients to age in place.

Responsibilities:

1. Make outpatient visits to clients within designated geographic territories.
2. Perform diagnostic tests and measurements, including mobility, joint range of motion, transfer status, stability, ambulation patterns, muscle strength and endurance, balance, and safety assessments.
3. Develop and implement individualized care plans, incorporating manual therapeutic exercises, gait training, balance, and other interventions.
4. Assess and revise care plans regularly, collaborating with nursing staff as needed.
5. Educate clients and their families on rehabilitative activities to promote health, safety, and independence.
6. Document observations, interventions, and evaluations accurately using a touch pad tablet.

Qualifications:

• Current VA Physical Therapist (PT) license.
• At least one year of recent PT work experience.
• Graduation from an American Physical Therapy Association-approved program or equivalent.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficiency with computers; experience with electronic medical records (EMR) preferred.
