Job summary

Chapters Health System is seeking an Admission RN to assess patients in their residence or hospitals and coordinate seamless transitions to Care Navigation. The role requires strong clinical assessment, effective communication, and the ability to manage multiple priorities with compassion.

Qualifications include an RN license in the relevant state, at least one year of nursing experience (hospice or hospital preferred), and experience with EMR/EHR systems.

Qualifications

• Current RN license in the state where the employee will be working.
• Minimum of one year nursing experience; hospice or hospital experience preferred.
• Experience with EMR/EHR systems.
• Strong clinical assessment skills; emergency department or ICU background preferred.
• Ability to communicate in English (verbal and written); bilingual in Spanish preferred.

Responsibilities

• Complete clinical admissions for all services and coordinate with Care Navigation.
• Explain services to patients/families and address questions empathetically.
• Obtain physician orders and begin Plan of Care.
• Provide skilled nursing interventions to prevent complications and maximize comfort.
• Document according to policy and liaise with hospitals, physicians and referrals.
