Job summary

Chapters Health System is seeking an Admission RN to assess patients at home or in hospital settings and initiate care transitions to Care Navigation. The role emphasizes compassionate patient interactions, timely documentation, and effective coordination with care teams.

The ideal candidate has an active RN license in the state, a current AHA/Red Cross BLS certificate, and at least 1 year of RN experience, with hospice or hospital exposure preferred. A valid driver's license is required.

Qualifications

• Current RN license in the state of employment.
• Active BLS for healthcare professionals.
• Minimum of 1 year RN experience; hospice or hospital preferred.
• Strong clinical assessment skills; ED/ICU background preferred.
• Experience with EMR/EHR systems.
• Ability to communicate in English; bilingual in Spanish preferred.

Responsibilities

• Explain services to patients/families and address questions empathetically.
• Identify admission objections and respond to concerns.
• Educate patients/families on admission requirements and needs.
• Complete informed consents and initial assessments per policy.
• Initiate Plan of Care and communicate with physicians.
• Perform skilled nursing interventions to prevent complications and improve comfort.
• Obtain physician orders and complete documentation.
• Coordinate with Care Navigation and facilities for referrals.
• Maintain SBAR-based clinical communication and timely updates.
• Support timely discharge planning and post-visit follow-up.
