Qualifications

• Licensed counselor with appropriate state licensure and credentials.
• Ability to comply with federal/state regulations and CARF guidelines.

Responsibilities

• Provide orientation to new patients to program rules, policies, and rights.
• Conduct intakes, discharge planning, initial assessments, and initial treatment plans within required guidelines.
• Involve patients in assessment, treatment planning, and intervention to support recovery.
• Administer biopsychosocial assessments covering addictions, medical and mental health history, socioeconomics, and legal factors.
• Develop and implement individualized treatment plans using SNAP strengths, needs, abilities, and preferences.
• Monitor and document changes in patient circumstances and address relapse indicators early.
• Adhere to Medical QA and Clinical Risk Management policies; assess high-risk conditions and administer screening tools (BAM-R, C-SSRS, GAD-7, PHQ-9).
• Coordinate with medical staff and other practitioners for referrals when needed; ensure proper communication and documentation.
