Job summary

Pinnacle Treatment Centers is seeking a Substance Abuse Counselor to oversee a patient’s treatment from admission through discharge. You will provide comprehensive psychological therapy, coordinate care, and support family and group counseling as part of a multidisciplinary team.

Qualifications

• Licensed in Indiana with LCAC, LCSW, LMHC, or LMFT.
• Minimum three years’ experience in substance abuse, mental health, or related field.
• Strong ability to coordinate care from admission to discharge and follow-up.

Responsibilities

• Interface with the clinical team in decision-making for patients, attending clinical meetings.
• Assess the counseling needs of the patient and determine appropriate level of care per ASAM criteria.
• Provide substance abuse counseling services per the patient treatment plan.
• Develop discharge/continuum of care plans to ensure movement to appropriate levels of care.
• Document counseling, assessments, and referrals in the clinical record.
• Maintain patient caseload and assist with case management and charting as required.
• Coordinate support services for patients and their families.
