You will provide individual and group counseling, coordinate referrals, and maintain accurate documentation within EHR systems. The position supports professional growth through a structured counselor tier program, continuing education opportunities, and comprehensive

Qualifications

• Associate degree or higher with appropriate counseling certification.
• Registration/certification by a recognized certifying organization.
• 0-2 years of counseling experience (Counselor I).
• Bachelor's degree requirement as applicable in some roles.

Responsibilities

• Provide orientation to new patients on program rules and rights.
• Conduct intakes, discharge planning, assessments, and initial treatment plans.
• Engage patients in assessment and treatment planning for recovery.
• Administer biopsychosocial assessments at intake and ongoing.
• Develop individualized treatment plans using SNAP framework.
• Monitor patients and document changes; address relapse indicators.
• Ensure compliance with QA and risk management policies and procedures.
• Provide counseling and referrals to second-tier services and community groups.
• Schedule counseling sessions and document visits in EHR.
• Engage in team meetings, trainings, and data collection activities.
• Perform other duties as assigned.
