Job summary

Pinnacle Treatment Centers is seeking a dedicated Counselor to support the recovery journeys of patients. The role emphasizes orientation, intake, assessments, and collaborative treatment planning to align care with individual needs.

You will administer standardized screening tools, monitor patient progress, and coordinate referrals to community resources as part of a holistic treatment approach.

Qualifications

• Experience providing professional counseling and referral services.
• Ability to conduct biopsychosocial assessments and develop individualized treatment plans.
• Knowledge of relapse prevention strategies and safety protocols.

Responsibilities

• Provides orientation to new patients to the programs’ rules, policies, procedures, and patients' rights.
• Conduct Intakes, Discharge Planning, Initial Assessments, and Initial Treatment Plans professionally and within time guidelines.
• Involve patients in assessment and treatment planning to promote engagement in recovery.
• Administer biopsychosocial assessments and evaluate patients’ lives in context of treatment.
• Develop and implement Treatment Plans using SNAP insights and clinical necessity.
• Monitor and document changes in patient status and address relapse indicators early.
• Comply with Medical QA and Clinical Risk Management policies.
• Provide counseling and referral services, coordinating with physicians and community resources.
