Job summary

Advocate Health is seeking a physical therapist to join our rehabilitation team. You will conduct musculoskeletal, neuromuscular, cardiopulmonary, and integumentary evaluations, establish and implement individualized plans of care, and monitor patient progress.

Qualifications

• Doctorate degree in Physical Therapy from an accredited school; licensure required.
• Excellent communication, interpersonal, and organizational abilities.
• Demonstrates ability to maintain productivity and adhere to billing/documentation policies.

Responsibilities

• Interprets physician referrals and completes musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations.
• Establishes and implements a plan of care; monitors outcomes and adjusts treatment as needed.
• Plans for discharge including home programs and family instruction; coordinates with care team and community services.
• Documents care and progress per department policy, including evaluations, progress notes, and discharge summaries.
• Maintains site productivity standards and adheres to billing and documentation timelines.
• May supervise PT assistants and rehab staff per state compliance and department policy.
