Job summary

HonorHealth is seeking a Physical Therapist to evaluate and treat individuals with physical disabilities, coordinating care with an interdisciplinary team across multiple Phoenix-area centers. Responsibilities include implementing tailored treatment plans, documenting progress, and collaborating with physicians, case managers, and rehab staff.

Qualifications

• Bachelor's in Physical Therapy from an APTA-approved university.
• Masters or Doctorate in Physical Therapy from an APTA-approved university (preferred).
• Current AZ Physical Therapy license in good standing or PT Compact license.
• Basic Life Support (BLS) certification required.
• Six months recent float/PRN experience preferred.
• Strong customer service and communication skills.

Responsibilities

• Implements individualized, evidence-based therapy plans.
• Evaluates patients with clear goals and discharge planning.
• Maintains accurate documentation of evaluations, progress, and charges.
• Collaborates with interdisciplinary team, physicians, and case managers.
• Provides patient and family education to support goals and outcomes.
