Job summary

The role includes discharge planning with home programs, patient teaching, and equipment referrals, along with comprehensive documentation and adherence to department policies.

Qualifications

• Bachelor's Degree in Physical Therapy is required.
• Wisconsin Physical Therapist license is required.
• Basic Life Support (BLS) certification within 6 months or per department policy.
• Excellent communication and interpersonal skills.

Responsibilities

• Interprets physician referrals and assesses musculoskeletal, neuromuscular, cardiopulmonary, and integumentary systems.
• Establishes a plan of care and implements treatment based on evaluation results and goals.
• Monitors outcomes and adjusts plans; provides status updates to physicians.
• Plans discharge including home programs, patient/family teaching, referrals, and equipment.
• Completes documentation per department policy (notes, reports, discharge summaries).
• Cleans up after treatment and maintains a neat, safe area; assists with lifting and moving as needed.
• Demonstrates use of modalities and equipment with knowledge of indications/contraindications.
• Participates in performance improvement activities and manages caseload; coordinates coverage.
• Pursues professional growth and orients new staff; participates in evaluations.
