Advocate Aurora Health is seeking a Physical Therapist to join a float team serving outpatient clinics within 45 minutes of home. The role emphasizes evaluating patients, planning care, and coordinating with physicians while maintaining documentation and quality of care.

Qualifications

• Bachelor's Degree in Physical Therapy required.
• Wisconsin or Illinois Physical Therapist license required.
• BLS for Healthcare Providers certification within 6 months.

Responsibilities

• Interprets referrals and conducts musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations.
• Develops and implements plans of care with functional goals.
• Monitors outcomes and updates treatment plans; communicates with physicians.
• Completes discharge planning with home programs and education.
• Documents progress notes, reports, and discharge summaries per policy.
• Assists with coverage based on department and regional needs.
• Maintains patient safety and uses modalities and equipment per orientation.
