Advocate Health in Wisconsin seeks a licensed Physical Therapist to evaluate musculoskeletal, neuromuscular, cardiopulmonary, and integumentary conditions and develop individualized plans of care. You will work with physicians and teammates to monitor outcomes and adjust treatment accordingly.

You will discharge patients with home programs, provide teaching, and document care per policy while maintaining productivity standards and complying with safety and infection control measures.

Competenze

• Doctorate degree in Physical Therapy from an accredited school.
• Maintains continuing education requirements as mandated by applicable state standards.
• Licensed Physical Therapist in the state where you work.
• BLS for Healthcare Providers certification (American Heart Association) required within 90 days unless exempted.

Mansioni

• Interpret physician referrals and perform musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations.
• Establish and implement plan of care based on evaluation results and goals; adjust as needed.
• Monitor outcomes and communicate progress to physicians and teammates.
• Discharge planning with written home programs and patient/family teaching; coordinate transitions in care.
• Document care per department policy; maintain productivity and safety standards.
• May supervise physical therapy assistants and rehab staff; provide guidance and training.
