Job summary

Advocate Health is seeking a Physical Therapist to evaluate patients, establish individualized treatment plans, and monitor outcomes. You will discharge patients with home programs and family teaching, while documenting progress and coordinating with physicians.

The role requires a Physical Therapy license in Wisconsin or Illinois, BLS, and strong communication, organization, and problem-solving skills. You will work in a collaborative setting with ongoing professional development.

Qualifications

• Bachelor's Degree in Physical Therapy.
• Licensed Physical Therapist in Wisconsin or Illinois.
• Basic Life Support (BLS) certification within 6 months.
• Excellent communication and interpersonal skills.
• Demonstrated organization, decision-making, and problem solving abilities.
• Team members will maintain annual educational requirements for hospital based certifications.

Responsibilities

• Interpret referrals and complete musculoskeletal, neuromuscular, cardiopulmonary, and integumentary evaluations.
• Establish a plan of care and implement individualized treatment based on evaluation results.
• Monitor outcomes and adjust treatment plans; communicate progress to physicians.
• Plan for discharge including home programs and patient/family teaching.
• Complete documentation per policy, including progress notes, reports, and discharge summaries.
• Maintain cleanliness and safety of treatment area and equipment.
• Demonstrate modality knowledge and adjust treatments based on indications/contraindications.
• Participate in performance improvement activities and staff education.
• Provide age-appropriate care and contribute to departmental standards.
