Angels Care Home Health is seeking an Account Executive to drive referral growth and admissions, building strong partnerships with physicians, hospitals, SNFs, and other providers. You will play a pivotal role in converting referrals into patient admissions and expanding our home health services across the territory.

The successful candidate will possess healthcare sales experience, strong relationship-building skills, and a solid understanding of post-acute care and payer sources.

Qualificações

• Proven success in healthcare sales, preferably within home health, hospice, or post-acute care.
• Strong understanding of home health eligibility, referral processes, and payer source requirements.
• Excellent communication, presentation, and interpersonal skills.
• Ability to work independently, manage time effectively, and succeed in a fast-paced environment.
• Valid driver’s license with a reliable transportation.

Responsabilidades

• Develop and maintain strong relationships with referral sources across the territory, including physicians, discharge planners, case managers, and community partners.
• Present and promote home health services, emphasizing patient eligibility, payer source options, and seamless transitions of care.
• Identify new business opportunities and convert them into active, ongoing referral partnerships.
• Educate healthcare providers on the benefits and appropriate use of home health, ensuring understanding of coverage, regulations, and admission criteria.
• Collaborate closely with clinical and operations teams, following established intake workflows, follow-up processes, and branch standards to ensure smooth admissions and patient satisfaction.
• Attend case conferences, focus meetings, and other required meetings as assigned.
• Consistently meet or exceed monthly admission goals.
