Job summary

Angels Care Home Health is seeking an Account Executive to drive referral growth, convert referrals to admissions, and build partnerships with physicians, hospitals, and post-acute providers.

The role requires strong relationship-building, knowledge of post-acute care, payer sources, and the ability to connect patients with high-quality home health services. Drive monthly goals in a fast-paced setting.

Qualifications

• Proven success in healthcare sales, preferably in home health, hospice, or post-acute care.
• Strong understanding of home health eligibility and payer sources.
• Excellent communication, presentation, and interpersonal skills.

Responsibilities

• Develop and maintain relationships with referral sources across the territory including physicians, discharge planners, case managers, and community partners.
• Present and promote home health services, focusing on eligibility and payer source options.
• Identify new business opportunities and convert them into formal referral partnerships.
• Educate providers on benefits, coverage, and admission criteria for home health.
• Collaborate with clinical and operations teams to ensure smooth admissions and patient satisfaction.
• Attend case conferences and required meetings as assigned.
• Meet or exceed monthly admission goals consistently.
• Maintain accurate CRM records and report outcomes regularly.
