Angels Care Home Health is seeking an Account Executive (AE) to drive referral growth and admissions across our home health network. You will build relationships with physicians, hospitals, and post-acute providers to connect patients with high-quality services.

Qualifications

• Proven success in healthcare sales, preferably in home health, hospice, or post-acute care.
• Strong understanding of home health eligibility, referral processes, and payer source requirements.
• Excellent communication, presentation, and interpersonal skills.
• Ability to work independently, manage time effectively, and succeed in a fast-paced environment.

Responsibilities

• Develop and maintain strong relationships with referral sources across the territory, including physicians, discharge planners, case managers, and community partners.
• Present and promote home health services, emphasizing patient eligibility, payer source options, and seamless transitions of care.
• Identify new business opportunities and convert them into active, ongoing referral partnerships.
• Educate healthcare providers on the benefits and appropriate use of home health, ensuring understanding of coverage, regulations, and admission criteria.
• Collaborate with clinical and operations teams to ensure smooth admissions and patient satisfaction.
• Attend case conferences and required meetings.
• Consistently meet or exceed monthly admission goals.
• Maintain accurate records in CRM systems and report outcomes.
