Job summary

Recruit Express Pte Ltd in Singapore is seeking a Virtual Assistant Care Coordinator in Healthcare, based in Jurong. This 8-month contract offers up to $3,500/month and standard hours Mon–Thu 8:30am–6:00pm, Fri 8:30am–5:30pm.

You will conduct patient outreach, support programme enrolment, coordinate with clinical teams, manage dashboards, and ensure data accuracy across IT systems to improve patient engagement and care continuity.

Responsibilities

• Conduct outreach to eligible patients and support programme enrolment
• Monitor the patient dashboard daily and review responses received through the patient-facing chatbot
• Route and upscale patient responses to the appropriate clinical team according to established workflows and protocols
• Follow up with non-responsive patients and guide them on using the chatbot
• Verify patient enrolment and ensure information is accurately reflected on the dashboard
• Communicate professionally with patients and caregivers through telephone and digital channels
• Coordinate appointments, referrals, handovers and communication between patients and clinical teams
• Recognise potential red flags and elevate concerns promptly according to established protocols
• Maintain accurate and timely documentation of patient interactions, outcomes and escalations across relevant IT systems
• Follow up on cases and ensure required actions are completed
• Maintain and update patient databases accurately
• Collate and validate data relating to enrolment, response rates, outreach attempts, escalations and patient outcomes
• Analyse data to identify trends, care gaps and recurring non-response patterns
• Prepare regular operational reports and updates
• Support audits, service evaluation and continuous improvement initiatives
• Use data to identify opportunities to improve workflows and patient engagement
