Job summary

Raffles Hospital in Singapore is seeking a Care Coordinator to support patients across the clinic network. You will coordinate appointments, referrals and follow-up care with doctors and providers to ensure continuity and a positive patient experience.

In this role you will manage patient journeys, liaise with clinics, hospitals and care teams, track investigations and recall activities, and assist with registrations and administrative duties as needed.

Qualifications

• Diploma, GCE A-Level or equivalent.
• Good communication and interpersonal skills with a patient-focused mindset.
• Proficient in English; knowledge of additional languages or dialects would be advantageous.
• Organised and able to manage multiple patient follow-ups effectively.
• Prior experience in healthcare, customer service or a service-related environment would be advantageous.

Responsibilities

• Coordinate and monitor patients’ care journeys, including appointments, investigations, referrals and follow-up care.
• Liaise with patients, doctors, clinics, hospitals and other healthcare providers to facilitate timely care and follow-up.
• Track outstanding investigations, referrals and abnormal screening findings, and coordinate the necessary follow-up actions.
• Conduct patient recall and engagement activities for health screening and related services.
• Provide patients with information and guidance on healthcare services and care pathways.
• Address patient enquiries and support service recovery where required.
• Maintain accurate patient records and care coordination documentation.
• Support clinic operations, patient registration and administrative duties when required.
