Job summary

Recruit Express Pte Ltd is seeking a contracted patient engagement role in the West of Singapore for 8 months. You will lead outreach to patients, monitor the dashboard, and route responses to the CDVC clinical team as per workflows. The role requires timely enrolment verification, data entry and coordination across clinical teams.

You will also maintain patient databases, coordinate appointments, document interactions and prepare reports to support audits and continuous improvement initiatives.

Qualifications

• Diploma in health sciences or relevant field.

Responsibilities

• Outreach to patients to seek enrolment.
• Daily monitoring of the dashboard and reviewing patient responses for routing/escalation.
• Contact non-responsive patients and guide responses via chatbot.
• Verify eligible patients are enrolled and reflected on the dashboard.
• Update and maintain patient database timely.
• Coordinate appointments, referrals and handovers across clinical teams.
• Ensure accurate documentation of patient interactions and outcomes across IT systems.
• Collate and validate data: enrolment, responses, escalations and outcomes.
• Analyse data to identify trends and care gaps; propose improvements.
• Prepare regular reports and updates; support audits and continuous improvement initiatives.
