As a Community Care Associate for our Fun with Friends Programme, you are a dedicated care team that delivers a person‑centred care model for our seniors, including persons with dementia. We aim to promote seniors’ independence and maintain their cognitive and physical functions through multi‑modality group programmes, ensuring they lead fulfilling lives.

With support and training from our team of professionals, you will continue to grow in your role. We are here to help you grow in your career and achieve your full potential in a nurturing and collaborative environment.

You will need to:

  • Facilitate various multi‑modality group activities for persons with dementia as assigned by the programme executive; including exercise sessions, meal preparation groups, cognitive games, music and art‑related activities.
  • Assist in activity planning and preparation to ensure the smooth execution of the group activities
  • Manage clients with different mental, and physical conditions and behaviour of concerns relating to dementia and assist in the monitoring of client’s health status
  • Assist in client’s care including providing necessary assistance for transfers, toileting and functional mobility
  • Assist in administering assessments to clients and caregivers, perform data collection
  • Perform timely documentation for the client post‑session
  • Assist to provide timely updates to family/NOK of client’s engagement
  • Support staff or volunteers relating to assist with running the programme
  • Assist in inducting new community care associates and provide necessary guidance
  • Assist in the procurement of activities materials, with approval sought from RO/Programme Executive
  • Perform general housekeeping to the programme premise
QUALIFICATIONS
  • Nitec / Diploma / Certificate in Healthcare or equivalent course
  • Some experience in social service or the healthcare industry would be an advantage
  • Facilitation skills and healthcare knowledge would be an advantage
OTHER INFORMATION

You should:

  • Be comfortable in facilitating group sessions and programme
  • Be empathetic and form positive relationships with persons with dementia and caregivers
  • Thrive in a collaborative and dynamic environment
  • Be proficient in English and Mandarin. The ability to converse in dialects will be an added advantage.
  • Be proficient in Google Suite Applications/MS Office
Working hours:
  • Five‑day work week 8-5.30pm. Timing may vary according to programme needs
  • Region‑wide- West, North and East region, but might be required to move around for role
