Job summary

Singapore General Hospital seeks a Right-siting Officer to support development and implementation of patient-centred care pathways, improving coordination across the hospital, public health institutions, and community providers.

The role collaborates with multidisciplinary teams to identify gaps in care processes, translate them into practical interventions, monitor adoption, and collect data to drive ongoing improvements. Office administration and patient support may be part of the duties.

Qualifications

• IT savvy and proficient in using Microsoft Office (Word, Excel and PowerPoint).
• Nursing / patient service / healthcare background will be an added advantage.

Responsibilities

• Conduct on-the-ground observations of clinical and operational workflows to understand the patient journey and identify service gaps.
• Translate identified gaps into practical workflow enhancements, patient-support interventions and referral solutions.
• Support the incorporation of new services into existing clinical workflows to facilitate appropriate and timely referrals.
• Monitor the implementation of right-siting initiatives and identify opportunities for further improvement.
• Collate and maintain data for care pathways rolled out.
• Any other adhoc duties as assigned.
• Right-siting counselling of patients and right-siting administration may be included in the long run.
