Singapore General Hospital seeks a Right-siting Officer to support development and implementation of patient-centred care pathways, improving coordination across the hospital, public health institutions, and community providers.
The role collaborates with multidisciplinary teams to identify gaps in care processes, translate them into practical interventions, monitor adoption, and collect data to drive ongoing improvements. Office administration and patient support may be part of the duties.
The Right Siting Officer supports the development and implementation of patient-centred care pathways that improve coordination within the hospital, between public healthcare institutions and with community providers.
The officer works closely with multidisciplinary clinical and operational stakeholders to identify gaps in existing care processes, develop practical interventions, improve patient education and facilitate patients' transition to appropriate community-based services. This includes conducting on-the-ground observations, gathering operational insights, addressing barriers to referral and appointment actualization, and supporting patients throughout the right-siting journey.
Your job roles will include, but not be limited to the following:
Conduct on-the-ground observations of clinical and operational workflows to understand the patient journey and identify service gaps.
Translate identified gaps into practical workflow enhancements, patient-support interventions and referral solutions.
Support the incorporation of new services into existing clinical workflows to facilitate appropriate and timely referrals.
Monitor the implementation of right-siting initiatives and identify opportunities for further improvement.
Collate and maintain data for care pathways rolled out.
Any other adhoc duties as assigned.
Right-siting counselling of patients and right-siting administration may be included in the long run.
Minimum GCE 'O' level and above
IT savvy and proficient in using Microsoft Office (Word, Excel and PowerPoint)
Prior experience in office administration will be valuable
Nursing / patient service / healthcare background will be an added advantage
