A leading recruitment agency in Singapore is seeking a Social Service Professional to conduct comprehensive psychosocial assessments and deliver counselling support. The ideal candidate will have a degree in Social Service-related fields, strong communication skills, and prior experience in healthcare or eldercare settings. Responsibilities include collaborating with multi-disciplinary teams, facilitating family meetings, and coordinating with community organizations. Conduct comprehensive psychosocial and financial assessments, considering mental health status, family relationships, and socio-economic conditions.
Deliver counselling and emotional support to patients and caregivers.
Provide psychoeducation on coping strategies, illness adjustment, grief processes, and major life transitions such as disability or end-of-life care.
Collaborate with Multi-Disciplinary Teams (MDTs) to develop, review, and implement personalized care plans.
Facilitate family meetings to support communication, care planning, and shared decision-making.
Act as a key liaison among families, community organizations (e.g., VWOs, support groups), government agencies (e.g., AIC, MSF, HDB), and internal hospital/clinic teams to coordinate resources and services.
Identify potential risks proactively, such as caregiver fatigue, elder abuse, or self-neglect.
Implement timely to reduce harm, including referrals and activation of community-based support.
Maintain accurate and detailed documentation of assessments, interventions, and case outcomes.
Ensure adherence to professional practice guidelines and organizational policies.
Contribute to service improvement initiatives, audits, and program evaluations.
Uphold ethical practice in alignment with the SASW Code of Ethics.
Min Degree in Social Service-related fields.
Strong communication skills.
Prior experience in healthcare or eldercare setting is a plus point!
