Responsibilities
  • Conduct comprehensive psychosocial and financial assessments, considering mental health status, family relationships, and socio-economic conditions.

  • Deliver counselling and emotional support to patients and caregivers.

  • Provide psychoeducation on coping strategies, illness adjustment, grief processes, and major life transitions such as disability or end-of-life care.

  • Collaborate with Multi-Disciplinary Teams (MDTs) to develop, review, and implement personalized care plans.

  • Facilitate family meetings to support communication, care planning, and shared decision-making.

  • Act as a key liaison among families, community organizations (e.g., VWOs, support groups), government agencies (e.g., AIC, MSF, HDB), and internal hospital/clinic teams to coordinate resources and services.

  • Identify potential risks proactively, such as caregiver fatigue, elder abuse, or self-neglect.

  • Implement timely to reduce harm, including referrals and activation of community-based support.

  • Maintain accurate and detailed documentation of assessments, interventions, and case outcomes.

  • Ensure adherence to professional practice guidelines and organizational policies.

  • Contribute to service improvement initiatives, audits, and program evaluations.

  • Uphold ethical practice in alignment with the SASW Code of Ethics.

Requirements
