Job description
Role Overview

The Medical Social Work Assistant supports the daily operational, administrative, and documentation needs of Ihsan Kidney Care (IKC). The role ensures smooth coordination of pre-admission processes, accurate record-keeping, timely data entry, and essential administrative support to maintain efficient service delivery for patients and the MSW team.

A. Care Coordination & Patient Management
1. Patient Coordination & Care Transition
  • Assist MSW to coordinate referrals and facilitate seamless transition of patients from hospitals to all IKC branches.
  • Support patients through key stages of their care journey, including admission, adjustment to the centre, and ongoing administrative needs.
  • Provide timely updates to stakeholders on acceptance status, assessment outcomes, and admission timelines.
2. Patient Enrolment & Follow-up
  • Assist in patient onboarding, including preparation of meeting room for admission counselling.
  • Assist MSW in basic financial counselling related to community dialysis subsidy schemes, payment obligations, and support options.
  • Assist with transport arrangements for new referrals or existing patients when required.
3. Multidisciplinary Collaboration
  • Work closely with the MSW together with the nursing and administrative team to uphold patient – centred care standards.
B. Direct support to Medical Social Work (MSW) Department
1. Administrative & Record Management
  • Prepare admission documents and support onboarding workflows.
  • Ensure timely and accurate updates in Electronic Medical Records (EMR) and shared internal systems.
  • Maintain logs for referrals, admissions, discharges, withdrawals/rejections, and key performance indicators (e.g., turnaround time).
2. Patient Engagement & Education
  • Support MSW in outreach, roadshows, and health awareness initiatives led by MTFA and IKC.
3. Financial Counselling & Psychosocial Support
  • Assist in financial counselling for pre-admission (MOH subsidies, Medisave, MediShield Life, funding schemes).
  • Assist in obtaining documents from patients and families for financial assessments, aid applications, and other schemes.
  • Assist in patient interviews, family conferences, mediation, and coordination of community resources.
4. Data, Reporting & Quality Assurance
  • Assist in audit compliance with organisational, MOH, and MOHH regulatory requirements.
C. Operational Support to Ihsan Kidney Care
1. Transport Coordination
  • Support MSW and Nursing team in scheduling transport services as needed for new and existing patients.
D. General Responsibilities
  • Uphold confidentiality of patient records and organisational information.
  • Perform any other duties assigned by the Reporting Officer or management to ensure smooth operations of IKC centres
Candidate Requirements
  • Minimum NITEC qualification (fresh graduates are welcome to apply)

  • Background or interest in Social Work, Community Work, or related fields will be an advantage

  • Good interpersonal and communication skills, with a willingness to learn

  • Meticulous, responsible, and able to work well in a team
