RESPONSIBILITIES

5 days work week, Monday to Friday

Working hours: 9 am to 6 pm

Able to travel around Singapore

Community Care Associate (CCA) provides skills training and intervention to Persons with Intellectual Disabilities (PwIDs) to enhance their independent living skills and participation in the community. The CCA performs person‑centred training and therapy according to the care/training plans developed by social service and/or healthcare professionals under Community Based Support Services (CSS). He/She is responsible for providing quality interaction and care to client.

Community‑Based Support Services (CSS) is the dynamic and nimble service delivery arm in MINDS that responds to emerging needs of PwIDs and Caregivers in the community. Other services include Me Too! Club, Future Care Planning Services, Caregivers Support Services, Special Student Care Centres, and Disability Case Management Programme, MINDS Clinic to enhance community inclusion and participation of caregivers and PWIDs in the community.

CCA plays a pivotal role in the training and well‑being of PwIDS. Using a person‑centred approach, he/she works towards improving their independent living and confidence to enhance client's quality of life. He/she maintains up‑to‑date documentation of clients’ conditions and learning progress.

He/She regularly provides inputs for the development and review of care plans. A caring, respectful, and patient professional, he/she should also be proactive, responsible, accountable, agile and adaptable to different work situations.

CCA also works closely with caregivers and PwIDs and build up client's and family support network with the support of social workers and community partners.

Provide Client Training and Support
  • Provide personal care assistance to clients in enhancing their daily and community living skills.
  • Support Allied Health Professionals (AHP) in providing assistance to clients in managing their rehabilitative needs.
  • Perform activities according to care/training plans.
  • Provide inputs for development and review of care plans.
  • Maintain up‑to‑date documentation of clients' conditions and progress.
  • Work with Multi‑Disciplinary Team to conduct weekly reviews and discuss concerns and progress of clients observed during sessions.
Develop Care Plan, training resources and updated documentation
  • Responsible for assessing clients' functional levels, contributing to individualised care and training plans, and participating in case reviews and evaluations.
  • Develop and maintain appropriate training activities and resources, while monitoring the effective implementation of care plans to ensure quality and consistency of care delivery.
  • Ensure timely, accurate documentation of clients' progress, care plans, and incident reports in line with organisational standards.
Perform Stakeholder Engagement
  • Build rapport and professional relationships with family caregivers and any community stakeholders e.g. volunteers/befrienders.
  • Communicate with caregivers to facilitate understanding of client's needs and intervention plans.
  • Guide client and caregivers on the techniques in performing self‑care and use of assistive devices to enhance learning.
  • Provide information to stakeholders on clients' progress.
Maintain order, safety and cleanliness of the care environment
  • Respond to unusual or challenging behaviours of clients.
  • Recommend measures to mitigate risk to clients with support of (AHP).
  • Follow advisories with regards to basic health, hygiene, and safety to provide a safe and healthy physical environment.
  • Ensure Training Materials is maintained in a clean and working condition.
  • Adopt proper documentation of incident reports.
  • Apply technology effectively for work and teaching.
Conduct programmes and communicate effectively with caregivers.
  • Conduct social, recreational and educational activities to promote independence, health, wellness, and quality of life.
  • Communicate with caregivers and work collaboratively on training plans.
  • Identify areas of improvements in programmes, activities and trainings.
QUALIFICATIONS
  • Minimum ITE certification.
  • Added advantage for those with experience in education/training of persons with intellectual disability / or multi‑disciplinary team environment.
  • Self‑motivated and fully able to participate in on‑site and off‑site activities.
