Job summary

Qualifications

• At least 2 years of experience as SSN I in a clinical setting.
• Ability to exercise good judgment and problem-solving skills.
• Good grasp of clinical workflow in Community Hospital and Long-Term Care settings.

Responsibilities

• Implement patient care in collaboration with the multi-disciplinary teams.
• Facilitate admissions, transfers, and discharges, ensuring timely discharge planning.
• Monitor patients for changes in condition and escalate issues as needed.
• Deliver patient education on disease progression and self-health care management.
